This page provides information on the types of forms Blaine Area Little League (BALL) needs to support registration and eligibility as required by the National Little League Association.

Annually, BALL must have a copy of each participant's

• birth certificate,
• school enrollment form/residency requirements, and
• medical release form each year.

Birth Certificate

To verify age and make sure your child is placed in the appropriate division, Little League requires that the league has a copy of each child's birth certificate. If you have not already uploaded your child's birth certificate to your account, please do so ASAP. School Enrollment Form/Residency Requirements

In order to verify that your child is eligible to play in our league, Little League must verify that you live within our league's boundaries. For more information, please see the Little League Residency Requirements page. This verification can take the form of the School Enrollment Form (signed by a school administrator, principal, or vice principal) or by uploading three separate documents confirming residency. Medical Release Form

To make it as easy as possible for our league members, we have implemented an online form to automatically fill out and digitally sign the Medical Release Form. This form needs to be filled out for each participant, each year. After filling out the form and receiving your PDF, please upload the PDF to your child's Participant Info. Please also print a copy of the form and turn it into your coach, even if you have scanned it into your account for this year. All coaches need a hard copy on hand at all activities, in case there is an emergency.
